I got lucky when I fell off a horse, I didn't black out. If I hadn't reacted very quickly I'd be dead right now. I think I was 16 at the time, the horse I was riding had been fixed too old, so he was pretty much the size of a stallion, without being one. His gimmick was that he'd trip himself to make people fall, well this time, he tripped himself and went down. As soon as I felt him go down I let go and threw myself back, so that he wouldn't fall on me.
Sadly my biggest worry at the time was not to end up face first in horse poo, since my boyfriend at the time was watching.
I don't think I've ever really had a "near death" experience before, but I've been convinced I was going to die before, many times. I'm one of the lucky people stuck with social anxiety. Whenever I have a panic attack, it feels like my brain is just screaming "you're going to die" at me and refuses to believe otherwise. It's amazing how much stuff can go through your mind at moments like that.

"Out there in the darkness, they're coming after me. A thousand stalking zombies, they want my brains to feed."
